Pharmacokinetics and Dosing Semaglutide exhibits a pharmacokinetic profile characterized by slow absorption following subcutaneous administration, enabling once-weekly dosing. After subcutaneous injection, the time to maximum plasma concentration (T_max) occurs approximately 1 to 3 days post-dose, with absolute bioavailability around 89% for the oral formulation when taken under fasting conditions.[72] Oral semaglutide absorption is markedly reduced by food intake, necessitating administration at least 30 minutes before the first meal or beverage of the day with no more than 4 ounces of plain water.[73] Dosing for type 2 diabetes with subcutaneous semaglutide (e.g., Ozempic) initiates with 0.25 mg subcutaneously once weekly for weeks 1–4, followed by 0.5 mg once weekly; if additional glycemic control is needed, increase to 1 mg once weekly after at least 4 weeks on 0.5 mg, and if needed, to 2 mg once weekly after at least 4 weeks on 1 mg (maximum recommended dose: 2 mg once weekly).
