Hydroxocobalamin is a naturally occurring cobalamin used medically as an injectable form of vitamin B12, characterized by its chemical formula C₆₂H₈₉CoN₁₃O₁₅P and molecular weight of 1346.4 g/mol.[1] As a naturally occurring cobalamin, it functions as a precursor to the active coenzymes methylcobalamin and 5-deoxyadenosylcobalamin, which are vital for red blood cell production, DNA synthesis, myelin synthesis, and metabolic processes including the conversion of homocysteine to methionine and methylmalonyl-CoA to succinyl-CoA.[2] It is primarily employed in medicine to address vitamin B12 deficiency and as an antidote for cyanide poisoning, where its central cobalt atom binds cyanide to form the nontoxic cyanocobalamin, facilitating urinary excretion.[1]
