Tiotropium bromide is a synthetic quaternary ammonium compound and long-acting muscarinic antagonist (LAMA) that functions as an inhaled anticholinergic bronchodilator, primarily indicated for the long-term, once-daily maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema, as well as for asthma in patients aged six years and older.[1][2] It is available in formulations such as dry powder inhalers (e.g., Spiriva HandiHaler, delivering 18 mcg per capsule) and soft mist inhalers (e.g., Spiriva Respimat 2.5 mcg per actuation for COPD [total daily dose of 5 mcg with 2 actuations] or 1.25 mcg per actuation for asthma [total daily dose of 2.5 mcg with 2 actuations]), with low systemic bioavailability due to poor gastrointestinal absorption, ensuring targeted action in the lungs; generic versions became available in the United States starting in 2023.[1][2][3]
