Ciclosporin, also spelled cyclosporine and cyclosporin, is a calcineurin inhibitor, used as an immunosuppressant medication. It is taken orally or intravenously for rheumatoid arthritis, psoriasis, Crohn's disease, nephrotic syndrome, eczema, and in organ transplants to prevent rejection. It is also used as eye drops for keratoconjunctivitis sicca. It is a cyclic peptide with chain length 11.

Ciclosporin is a potent immunosuppressive medication primarily used to prevent organ rejection in transplant recipients by inhibiting T-cell activation through calcineurin blockade.[1] Derived from the fungus Tolypocladium inflatum, it is a lipophilic cyclic undecapeptide that binds intracellularly to cyclophilin, forming a complex which inhibits the phosphatase activity of calcineurin, thereby preventing the dephosphorylation and nuclear translocation of nuclear factor of activated T-cells (NFAT), ultimately suppressing cytokine production such as interleukin-2.[2] This mechanism renders it non-myelotoxic but highly nephrotoxic, necessitating careful monitoring of renal function during therapy.[2]
Ciclosporin เป็นยากดภูมิคุ้มกันที่ออกฤทธิ์แรง ใช้หลักเพื่อป้องกันการปฏิเสธอวัยวะในผู้รับการปลูกถ่ายอวัยวะ โดยยับยั้งการกระตุ้นเซลล์ที (T-cell) ผ่านกลไกการปิดกั้น calcineurin[1] ได้มาจากเชื้อรา Tolypocladium inflatum เป็น cyclic undecapeptide ที่ชอบไขมัน (lipophilic) ซึ่งจับกับ cyclophilin ภายในเซลล์ ก่อตัวเป็นสารเชิงซ้อน (complex) ที่ยับยั้งกิจกรรมเอนไซม์ฟอสฟาเทส (phosphatase) ของ calcineurin จึงป้องกันการดีฟอสฟอรีเลชัน (dephosphorylation) และการย้ายเข้าสู่นิวเคลียส (nuclear translocation) ของ nuclear factor of activated T-cells (NFAT) ในที่สุดจึงยับยั้งการสร้าง cytokine เช่น interleukin-2[2] กลไกนี้ทำให้ยาไม่เป็นพิษต่อไขกระดูก (non-myelotoxic) แต่มีความเป็นพิษต่อไต (nephrotoxic) สูง จึงจำเป็นต้องเฝ้าระวังการทำงานของไตอย่างใกล้ชิดระหว่างการรักษา[2]
