Carbamazepine is an anticonvulsant medication belonging to the class of iminostilbene derivatives, chemically related to tricyclic antidepressants, with the molecular formula C₁₅H₁₂N₂O.[1][2] It is primarily used to treat partial and generalized tonic-clonic seizures in epilepsy, either alone or in combination with other drugs, as well as trigeminal neuralgia and acute manic or mixed episodes associated with bipolar I disorder.[3] The drug exerts its therapeutic effects mainly by binding to voltage-gated sodium channels in their inactive state, thereby stabilizing neuronal membranes, inhibiting repetitive neuronal firing, and reducing excitatory synaptic transmission.[4][3]
Carbamazepine เป็นยากันชัก (anticonvulsant) ที่จัดอยู่ในกลุ่มอนุพันธ์ iminostilbene มีความสัมพันธ์เชิงเคมีกับยาแก้ซึมเศร้ากลุ่ม tricyclic antidepressants และมีสูตรโมเลกุล C₁₅H₁₂N₂O[1][2] ใช้เป็นหลักในการรักษาภาวะชักแบบบางส่วน (partial seizure) และภาวะชักทั่วไปแบบ tonic-clonic ในผู้ป่วยโรคลมชัก ทั้งการใช้เพียงตัวเดียวหรือร่วมกับยาอื่น รวมทั้งใช้รักษาอาการปวดประสาทไทรจีมินัล (trigeminal neuralgia) และภาวะฟุ้งพล่านเฉียบพลัน (acute manic) หรือภาวะผสม (mixed episode) ที่เกิดร่วมกับโรค bipolar I disorder[3] ยานี้ออกฤทธิ์ในการรักษาเป็นหลักโดยการจับกับช่องโซเดียมแบบขึ้นกับศักย์ไฟฟ้า (voltage-gated sodium channel) ในสภาพไม่แอคทีฟ (inactive state) จึงช่วยทำให้เยื่อหุ้มเซลล์ประสาทเกิดเสถียรภาพ ยับยั้งการยิงกระตุกของเซลล์ประสาทที่ซ้ำ ๆ และลดการส่งสัญญาณประสาทแบบกระตุ้น (excitatory synaptic transmission)[4][3]
