Insulin glargine is a long-acting recombinant human insulin analog designed to provide basal insulin coverage for up to 24 hours with a relatively steady glucose-lowering effect and no pronounced peak.[1] It is administered via subcutaneous injection once daily and is primarily used to improve glycemic control in adults and pediatric patients with type 1 diabetes mellitus, as well as in adults with type 2 diabetes mellitus.[2] Unlike short- or rapid-acting insulins, its modified structure—featuring glycine substituted for asparagine at position A21 and two arginines added to the C-terminus of the B-chain—allows it to precipitate at neutral pH in subcutaneous tissue, resulting in a slow, prolonged release.[1]
