Phenytoin, chemically known as 5,5-diphenylhydantoin, is an anticonvulsant medication primarily indicated for the management of epilepsy, including generalized tonic-clonic seizures, complex partial seizures, and status epilepticus, as well as for preventing and treating seizures during or following neurosurgery.[1][2] First synthesized in 1908 by German chemist Heinrich Biltz as a barbiturate derivative, its potent anticonvulsant properties were discovered in the late 1930s through systematic testing by researchers H. Houston Merritt and Tracy Putnam, who identified it as superior to existing therapies like phenobarbital for controlling major motor seizures without excessive sedation.[3][4] Phenytoin exerts its therapeutic effects by prolonging the inactive state of voltage-gated sodium channels in neuronal membranes, thereby stabilizing hyperexcitable cells and inhibiting the propagation of seizure discharges.[1] Marketed under brand names such as Dilantin, it revolutionized epilepsy treatment upon its introduction in 1938 but is characterized by nonlinear pharmacokinetics, a narrow therapeutic index, and potential for significant adverse effects including gingival hyperplasia, hirsutism, and hematologic disturbances, necessitating careful monitoring of serum levels.[5][1]
